Different people certainly have different issues under the eyes, but overall, concerns about the eyes or the area around the eyes is a top cosmetic concern. You may describe it as looking tired, having bags under the eyes, or dark circles. The area under the eye can look dark, especially because of volume loss, the skin can become crepey and inelastic, wrinkles can develop and you can develop puffiness. Also, you may notice loose, falling skin on the upper eyelids. It may start at any age in adulthood.

All of the reasons that you may see aging in your face/skin overall contribute to aging around your eyes. There is volume loss everywhere in the skin as you get older – less collagen production and more collagen destruction, and loss of fat and elastin. You may notice it first (and/or worst) around your eyes because 1) they are central to the face and what people look at most, and 2) the skin is really thin around the eyes naturally, so any changes are very noticeable. There is some genetic component to how and when we age around the eyes and it may start early (in the 20’s and 30’s) for some.
Aging around the eyes happens under the eyes (dark circles, volume loss, puffiness, crepey skin), above the eyes or on the upper eyelids (skin sagging and thinning), or to the side (crow’s feet/wrinkles). You may experience more or less of each of these signs of aging eyes.
How do we treat under eye bags?
Younger patients with healthy and youthful skin and only volume loss may be good candidates for volume replacement with fillers only, plus or minus injectable toxin to prevent wrinkles. However, as you age, you will need some procedures to stimulate collagen and elastin production to keep the eye skin tight, smooth and supple. As always, eye creams will help achieve and maintain your results.Â
ThermiSmooth is a non-invasive radiofrequency based treatment that smooths and tightens skin. Over the course of several treatments, the skin becomes tighter and smoother as you develop new collagen and elastic tissues. It is especially effective around the eyes to improve wrinkles beneath the eyes as well as lift and tighten the upper eyelid.
Ultherapy is an in-office procedure FDA approved to lift and tighten the skin. It uses ultrasound energy to stimulate the tissues to make new collagen and importantly, elastic tissue to lift and tighten skin. It can be especially helpful in improving puffy, loose, or aged-looking skin around the eyes.
Injectable fillers are used to add volume to areas of the skin where it is lacking. They can be added where fat, collagen or even bone has been lost or where you want to augment your appearance like to sharpen a jawline, to improve chin projection, to lift your cheeks or shape your nose. Today’s fillers are usually hyaluronic acids, but can also be collagen, calcium hydroxyapatite or your own fat.
Injectable toxins are better known as Botox or Dysport. They are injected into areas where lines, wrinkles, or folds are created from movement. Around the eyes, it is especially effective in improving the wrinkles around the eyes, in lifting the brows and smoothing out the skin around the eyes.
